Gastroenterology

Your first office visit

You will need to fill out three forms prior to your office visit. These are the Acknowledgement of notification of privacy form, information form and medical history form. Please click here to print out these forms. Please fill out these forms prior to your appointment. Please bring an updated medication list for your appointment. Please arrive at our office at the scheduled appointment time. You will check in at the front desk and submit your completed forms.


Waiting Area

After your check in is completed, you will have a seat in the waiting room until you are called into the patient room.

We have television as well as a variety of reading material in the waiting room for your convenience. We also have unrestricted Wifi capabilities throughout our facility for your convenience. Our doctor will see you shortly after you are checked into the patient room.


Patient Room

After your consultation with the physician has concluded, you will proceed to the check out desk. There, you will make future appointments, get paperwork for your labs and prescriptions if needed. You will also schedule endoscopic procedures, if required. You will receive written instructions for the preparation for your procedure, if needed. You can download and review the prep instruction here as well. You will be required to make a payment for any copay that your insurance may mandate. Your appointment will then conclude.
